For anyone else that may be having this problem…
Upgrading to the latest firmware fixed the problem.

Reading the release notes (wow what a concept… hehe).
There are a number of keyboard related fixes since the version I am running.
I have upgraded the OS and will see if that makes a difference.

Yes, original keyboards and mouse.
I have the firmware downloaded and am actually attempting an upgrade from my laptop right now. (On a separate note, FTP upgrade is telling me wrong version Z_Z)
I will try the ICA Client as well.

Running Version: 5.0 FR2 Build 563.5
I am being told that this only happens when they use ICA Connections.
Just checked the keyboard mappings and its set to US (tested all the keys and everything comes up just fine)
Haha well I tried playing around with ssfloat this afternoon trying to rip it but it was my understanding that a bmp was loaded and that it was in the contents of nt.bin (or whatever it was called).
Unfortunately I ran out of time and had to ship the equipment.
I have a lab client left and I will try that when I get back to work.

Pretty much it says when ssfloat.dll creates a new thread it looks in:
windowswindowsce.bmp
Unfortunately I have been able to find such a file or directory in flash.

Well I can certainly second this problem. I have a large number of V30 VX0s that I have been trying to upgrade… I have tried multiple software packages and run into the same error message.
Anyone have any clues on this?
